根据项目当前版本生成带明确版本号和日期的初始化记录；未知版本时先确认，不使用 Unreleased 作为版本号。
格式： ## [版本号] - YYYY-MM-DD → 分类 → 变更条目。
分类： 使用项目协议规定的分类；未规定时采用“新增 / 改进 / 修复 / 技术优化 / 文档完善”。
